PCL Reconstruction Surgery

PCL(Posterior cruciate ligament) is one of the crucial ligaments present inside the knee joint and cross each other to form an “X” with ACL(Anterior cruciate ligament) in front of the knee that runs obliquely inside the knee joint and connects the thigh bone (femur) to the lower leg bone (Tibia). Treatment Options

PCL injury can be treated and managed by one of the following treatment options depending upon the grade of injury.

  • The grades of PCL injury can be categorize as follows:
    • Grade I: The PCL has an incomplete tear
    • Grade II: The tendon is incompletely torn and is looser than in Grade I
    • Grade III: The tendon is totally torn and the knee winds up unsteady
    • Grade IV: The PCL is harmed alongside another tendon in the knee

  • Non-Surgical Treatment
    • Stabilization and Protective Bracing: In Acute or Chronic PCL tear, one can use tailor made knee braces and stabilizers to lessen the stress on the injured ligament and curbing the movement at knee and thereby aiding in healing process.
    • Conservative Management: This is indicated in cases of Acute and Chronic PCL tear, in which proper rest is advisable with the limited to no mobility in order to protect the joint and healing the ligament.

  • Surgical Treatment
    Treatment for PCL tear often depends on the grade of PCL tear or injury. If it is partial or complete tear with significant knee instability, then the patient may require surgical intervention. Surgery for PCL tear can be classified into two types:
    • PCL Repair Surgery: In this procedure, the partially or completely torn PCL ligament is repaired by sewing it back together using sutures. However, studies have shown that the PCL repair generally fails over the time and may require revision surgery or reconstruction.
    • PCL Reconstruction Surgery: In this procedure, the torn PCL ligament is replaced by using a substitute graft taken from either of the following sources:
      • Hamstrings Muscle Tendon AutoGraft taken from the patient's own knee
      • Patellar Tendon AutoGraft taken from the patient's own knee
      • Quadriceps Muscle Tendon AutoGraft
      • Allograft that is a graft taken from a cadaver’s, Thigh or Heels


During PCL reconstruction procedure the damaged ligament will removed and replaced by the surgeon instead of repairing it. The surgeon replaces the damaged ligament with the segment of tendon tissue analogous to a ligament that connects muscle to bone. The replaced tissue is termed as graft. A piece of tendon is used by the surgeon from your other knee or from a deceased donor.

This procedure is done under general anesthesia. Your surgeon will make a hole or excavate into your femur bone and tibia bone to precisely locate the graft which will protect your bones with screws. The graft will provide as gibbet on which new ligament tissue can grow.

After the procedure, when you will get back from anesthesia, on the same day you will be permitted to go home. Before going home your surgeon will ask you to put on a knee brace to shield the graft.
