Pituitary Tumor Treatment

A pituitary gland tumor is a group of abnormal cells that develop in pituitary gland. Most of the pituitary tumors are noncancerous (benign) growths (adenomas). Adenomas remain in pituitary gland or surrounding tissues and do not spread beyond the skull.

Pituitary tumors may affect the hormonal level that regulate important functions of our body. Some of them may result in increase in hormonal levels whereas some can cause pituitary gland to produce lower levels of hormones.

Pituitary gland is only about the size of a pea, but controls a lot of important functions of human body, like growth, heart rate, and ability to have kids. Moreover, the pituitary gland produces hormones that regulate critical body organs and glands, including the:

  • Thyroid gland
  • Adrenal glands
  • Ovaries
  • Testicles

It is also called asthe master gland because it tells our other glands when to make more hormones. These hormones then move throughout the body and tell our organs what they need to do to keep everything in working order.

There are various options for treating pituitary tumors, including medications to manage hormone level, removing the tumor and controlling its growth. Large pituitary tumors those measuring about 1 centimeter or larger are known as macroadenomas. Because of their size, macroadenomas can put pressure on the pituitary gland and nearby structures. Smaller tumors are called as microadenomas.

Symptoms of Pituitary Tumor

Symptoms experienced due to pituitary tumors depend on their size and the hormones they release. Some common symptoms include :

  • Headache
  • Vision loss
  • Seizures
  • Weight changes
  • Irritability, anxiety, and depression
  • Nausea and vomiting
  • Dizziness
  • Confusion
  • Flushing of the face
  • Weak muscles and bones
  • High blood pressure
  • Irregular heartbeat
  • Large hands and feet (acromegaly)
  • Producing breast milk even if not pregnant
  • Lack of milk when breast-feeding
  • Menstrual cycle changes
  • Lowered sex drive
  • Impotence
  • Abnormal sexual development and growth in children

Pituitary tumors often go undiagnosed because of their symptoms which resemble similar to other conditions. And sometimes pituitary tumors are found because of medical tests for other medical conditions.

To diagnose a pituitary tumor, doctor will likely take a detailed history and perform a physical exam or order below mentioned tests :

  • Blood and urine tests - These tests can determine whether one has an over production or deficiency of hormones.
  • Brain imaging - A CT scan or MRI scan of brain can help doctor judge the location and size of a pituitary tumor.
  • Vision testing -This can determine if a pituitary tumor has impaired sight or peripheral vision.

In addition, doctor might refer patient to an endocrinologist for more-extensive testing.

Pituitary Tumor Treatment

Many pituitary tumors do not need treatment. For those requiring treatment, options depend on type of tumor, its size and how far it has grown into brain. Age and overall health also are also important factors to consider. Treatment options include :

  • Medication or Drug Therapy - Drug therapies are used to shrink and destroy the tumor or decrease the amount of hormones the tumor is producing.
  • Surgery - Surgery removes the pituitary gland.
  • Radiation therapy - It is used to kill tumor cells.

These options are considered either alone or in combination, to treat a pituitary tumor and return hormone production to normal levels.

Surgery for Pituitary Tumor

Surgical removal of a pituitary tumor is necessary if the tumor is pressing on the optic nerves or the tumor is overproducing certain hormones. The two main surgical techniques for treating pituitary tumors are as below :

  • Endoscopic trans-nasal transsphenoidal approach: This approach usually enables the doctor to remove the tumor through nose and sinuses without an external incision. No other part of the brain is affected, and there's no visible scar. Large tumors may be difficult to remove this way, especially if a tumor has invaded nearby nerves or brain tissue.
  • Transcranial approach (craniotomy): In this approach tumor is removed through the upper part of the skull via an incision in scalp. It is easier to reach large or more-complicated tumors using this procedure.

After pituitarytumor surgery, it will take time for the patient to return to his/her usual level of energy. Regular checkups may be required post-surgery. Extra rest would be required for healing. Generally, patient will see the neurosurgeon two weeks after the surgery and the endocrinologist six weeks post-surgery. An MRI scan will usually be done around three months after surgery. The recovery period may vary ranging from 4-6 weeks to approximately 6 months.
