If you are considering the knee replacement surgery and have just started searching for the treatment options, then here are details which will help you understand more about the knee replacement surgery and shall help you make an informed decision.

Overview

Total Knee Replacement Surgery, commonly known as Total Knee Arthroplasty, is the specialized surgical intervention in which the impaired parts of the knee joint including the bone and cartilage are replaced with the artificial components. Knee is considering as one of the main weight bearing joint of the body which works as a fulcrum between the thigh bone (femur) and the lower leg bones (tibia & fibula). Over a period of time, the articulating surfaces of these bones wears off due to degenerative changes or other such factors and results is pain, discomfort, swelling and limited mobility. In such cases, Total knee replacement surgery is considered as a boon to relieve pain, restore the normal functions of the knee and enhance one’s quality of life.

Causes

As stated earlier, knee replacement is indicated in the individuals experiencing severe pain, discomfort and limited mobility at the knee joint. So, enumerated below are the most common causes for knee pain and discomfort.

  • Osteoarthritis:This is most familiar medical condition which is age related and causes wear and tear of the weight bearing knee joint over a period of time. The degenerative changes happens over an age of 40-50 years when the cartilage that surrounds the joint surfaces begins to wear away and bone starts becoming soft resulting in abrasion of the bones and developing symptoms like stiffness, joint pain, swelling, limited range of motion etc.

  • Rheumatoid arthritis: This is another prevalent type of arthritis which is auto immune in nature. In this type the synovial membrane that surrounds the joints get inflamed which in turn starts destroying the cartilage and underlying bone, thereby producing the symptoms.

  • Post Traumatic Arthritis: This usually happens post knee injury to the crucial structures surrounding the knee joint. For example, in cases like knee injury like fractures and ligament tear, there can be damage to the cartilage cushioning the knee joint bones which would ultimately result in symptoms related to arthritis.

Treatment Options

Treatment for Arthritis can include both non-surgical and surgical option depending upon the severity of the condition.

  • Non-Surgical Treatment
    • Medical Management: This includes ingestion of appropriate drugs like analgesics and anti- inflammatory medications in order to subside the symptoms
    • Knee Bracing: In cases, where the symptoms are due to inappropriate posture or mal-alignment of the bones, then knee bracing and stabilization can be an option in order to correct the knee alignment and equalizing the weight on both the knee joints.
    • Physiotherapy: This can be one of the options to help control the symptoms and improve the functionality of the knee joint.
    • Visco supplementation injections: This aims to lubricate the joint space to make the movement at the knee joint less painful

  • Surgical Treatment
The surgical treatment includes knee replacement surgery which has several types and variations. The most appropriate one is indicated for each patient depending upon the several factors including age, medical condition and severity of the disease. Following enlisted are the types of knee replacement surgery in India
    • Conventional Total Knee Replacement: This procedure is the indicated for the patients suffering from chronic arthritis and when the patient has tried all other non-surgical treatment option but the patient didn’t get relieved. In this, the damaged articulating surfaces of the knee joint are replaced with the artificial implants through an incision of around 7-8” around the knee joint.

    • Minimally Invasive Knee Replacement: This is the novel procedure also known as “Quadriceps- Sparing Knee Replacement”. In this, the knee replacement is done like the conventional one but the only difference is that it is done through smaller incision of just 3-4’’ which circumvents any injury or trauma to the quadriceps muscles. Thus it is known as Quadriceps Sparing Knee Replacement Surgery. Minimally Invasive Knee Replacement has many benefits including speedy recovery, smaller incisions, less hospitalization and also the patient is made ambulatory the very next day of the surgery.

    • Hi- Flex Knee Replacement: This is another important type of knee replacement surgery which allows full knee bending up to 155 degrees after surgery. However, not every patient is a candidate for hi-flex knee replacement. This degree of high flexion is attained only by selecting right patient, implant design, surgical technique and aggressive rehabilitation post-surgery.

    • Partial Knee Replacement: This is also known as uni-condylar r uni-compartmental knee replacement. In this, only the damaged compartment of the knee joint is replaced with the implant in spite or replacing the entire joint. This is indicated in very rare cases where patient is suffering from degenerative changes of the part of the knee joint.

    • Computer Navigated Total Knee Replacement: In this, the Knee Replacement surgery is performed using the most advanced and revolutionary -NAV3 navigation technology. This technique has resulted in improved accuracy of the prosthetic implantation which further results in less scarring due to smaller incision, quick recovery thereby increasing the short and long term performance of the knee joint.

Are you a Candidate?

You can be considered as the good candidate for Total Knee Replacement Surgery if

  • You are suffering from chronic and severe pain in knee joint
  • Stiffness at the knee joint
  • Stiffness at the knee joint
  • Knee Deformity or mal-alignment of the lower leg bones resulting in painful symptoms
  • If all other conventional treatment options have not be been fruitful in relieving the painful symptoms

Procedure

It is essential for you to know details of the surgical procedure you will be undergoing beforehand to prepare yourself for the surgery and cope with it for better recuperation. Your surgeon shall perform the knee replacement surgery in steps explained below:

Step 1: Making you sleep: The anesthetist shall give you either the general or spinal anesthesia to make you fall asleep during the procedure.

Step 2: Incising the knee: The surgeon shall make an incision over the anterior part of your knee which allows easy access to the patella often known as kneecap. The incision in typically about 8-10 inches in length in case of traditional knee replacement surgery. In contrast, the incision is only 4-6 inches long if the surgery is being performed using minimally invasive approach.

Step 3: Turning the Kneecap: After making incision over the knee, the first exposed part is your patella. As the surgeon gains access to patella, he rotates it outwards providing adequate room to perform the procedure.

Step 4: Preparation of Thighbone: The surgeon then resurfaces your thighbone medically known as femur. After carefully exposing the knee joint, the surgeon uses specialized instruments to measure the bone and to cut the damaged bone and cartilage from the base of the femur with utmost precision. The cut end is then resurfaced for appropriate fitting of the femoral component of the prosthesis.

Step 5: Fixing the Femoral component: As the femur is resurfaced, the surgeon fixes the femoral component made of metal or plastic to the base and fix it into place using bone cement.

Step 6: Preparation of Shinbone: Shinbone also called as Tibia is the next bone to get resurfaced. The upper end of the tibia is made free of damaged bone and cartilage and then it is reshaped so as to fit the tibial component of the artificial knee prosthesis.

Step 7: Fixing the Tibial component: The Tibial component (tibial tray) is then fixed to the upper resurfaced part of the tibia and is glued in place using bone cement. As soon as the tray is fixed, the surgeon places a polyethylene insert to convene between the femoral and tibial component that acts as a buffer and provides support to your body while bending and flexing your knee.

Step 8: Re-tuning the Patella:The surgeon smoothens the lower surface of the patella and fix it with the added plastic patellar component in order to certify appropriate fit with the rest of implant. If required, the plastic component is cemented to the bone.

Step 9: Concluding the Procedure: The surgeon then checks for the proper alignment, working, sizing and positioning of the implant by bending and flexing the knee. After confirming, the surgeon sutures the incision and dresses it. The surgery take approximately 2 hours to complete.
